To register, please print this application. A 50% deposit is required with the application. .Deposits are non-refundable. However, you may reschedule your training if you miss your class . Note: Please submit one form for each student.

If you want an ASA Study Book and Log Book then please include the cost for these items with your class deposit. Cost of Study Book varies with class type. To check cost please see Course Outlines. The cost for an ASA Log Book is $10.00 per student.
